LIFE TEEN Registration: High School Youth Ministry

We do ask all Breakaway and U.C. participants to register each year.  This allow us to have your correct contact information, and helps offset the cost of providing dinner on Sundays and snacks on Wednesday nights.  All registered youth will receive a youth ministry t-shirt (each year has a new design).  If you have questions, please contact David or Sue at 303-220-3388.  Please download the form below for the 2009-2010 ministry year and register today!
